On or about April 5, 2016, at 1:00 p.m., an employee slipped and fell 6 to 8 feet from the fixed industrial ladder to the boiler's eighth elevation platform, suffering compound fractures to the tibia and fibula, as well as metatarsal fractures in both feet. He was hospitalized overnight for surgery.
